The Nottingham West Primary Care Network (PCN) includes a range of organisations that work together to support the health and wellbeing of local people.
We cover the three Broxtowe neighbourhoods of Beeston, Eastwood and Stapleford.
Our network includes 12 GP practices, local authorities and social care services, community pharmacies, hospitals, mental health services, voluntary services and others.
Our aim is to help local people to live healthy and rewarding lives with high quality healthcare and support with issues such as social isolation, loneliness, education and employment opportunities and housing.
We are part of the South Nottinghamshire Place-Based Partnership, which is a collaboration of commissioners, health and care providers and local authorities working together to join up health and social care services across Broxtowe, Gedling, Rushcliffe and Hucknall (Ashfield). Our aim is to support people in South Nottinghamshire to live healthier lives and get the care and treatment they need, in the right place, at the right time.
