An article on the award-winning work of Nottingham West PCN’s pharmacy cardiology team has been published in the national Prescriber magazine.

The piece, ‘Hypertension: a PCN Clinical Pharmacist-led intervention’, highlights how the PCN developed and implemented a unique pharmacist-led pilot to find and proactively manage patients with hypertension. The service was designed to deliver a more convenient service for patients, to free up resources within practices, to increase Quality Outcomes Framework prevalence and to save lives.

A pharmacy-led team worked at scale across the PCN general practices and in close partnership with patients and community pharmacies to proactively identify those at risk of Cardiovascular disease (CVD) in line with the IIF targets.

Beth Rushton, Senior Clinical Pharmacist at the PCN (pictured above right with Clare Watson, Health Inequalities Lead), said: “CVD remains the second most common cause of death in England, after dementia and Alzheimer’s disease, and hypertension is the largest single known risk factor for CVD. Our article outlines how, as a PCN, we are working in new ways to make a difference to the health of our local population.”
